Mail the following documents, along with a postage-paid, self-addressed envelope,  to the Treasurer - Town of Sumpter Treasurer, PO Box 365, Baraboo, WI 53913:

1. Check made out to the Town of Sumpter for the appropriate amount from the Town of Sumpter Fee Schedule.
2. A copy of the statement from your vet showing the name, breed, and color of the dog.
3. Proof of rabies that includes the following information:
   • Name/address of the veterinary that provided the vaccination
   • Date of the shot
   • Expiration date of the shot
   • Name of the vaccine
   • Serial number or lot number
